  Please note: the agreement is provided for your reference only. You do not need to return a paper copy—only the digital form must be submitted.

  2026-2027 Volunteer Agreement/Copyright Assignment/Non-Disclosure Agreement/Conflict of Interest Form

  If you do not submit the online acceptance form, this will indicate that you do not accept the assignment and your chair will be informed accordingly. Please contact Lisa Gunderson at lgunderson@aapd.org if you have any questions.

  Note that multi-year and non-term-limited appointments must be reaffirmed each year by completing the volunteer form.
