  If you do not submit the online acceptance form, this will indicate that you do not accept the assignment and your committee chair will be informed accordingly. Please contact Lisa Gunderson at lgunderson@aapd.org if you have any questions.

  Note that multi-year and non-term-limited appointments must be reaffirmed each year by completing the volunteer form.
